United Corporate Services, Inc. (UCS) has compiled information regarding Article 9 and the July 2013 Amendments from our network of agents, jurisdictional websites and from our association with the International Association of Corporate Administrators (IACA). The information and charts below are updated when we received news of a change.
Article 9 of the Uniform Commercial Code (UCC) was revised in 2001. The code was amended in 2010 with an effective date in most states of July 1, 2013. Many states have not yet enacted these amendments so the need for 2 sets of UCC forms is required. Please note the differentiation.

UCC-3 Amendment form
UCC-1 Addendum The Addendum form is used to list up to two additional debtors or one additional secured party. Can be used to add an assignor secured party. Additional use for fixture/real-estate related filings.
UCC-3 Addendum This is used when amending fixture/real estate-related filings.
UCC-1-AP This form is used to list an Additional Party or parties up to three additional debtors and to two additional secured parties (for the UCC-1)
UCC-3-AP This form is used to list an Additional Party or parties up to three additional debtors and to two additional secured parties. (for the UCC-3)

UCC-3-AP This form is used to list an Additional Party or parties up to three additional debtors and to two additional secured parties. (for the UCC-3)
Co-op Addendum Form (NY) The form is utilized for a co-op filing making it effective for 50 years.
